As part of duties of Presiding officer, he/she has to fill some proformas besides conducting of polling process in polling booth. The presiding officer has to understand what kind of proformas given by the election commission of india to fill during the conduct of  polling. Accordingly,  time to time PO has to fill all the proformas provided to him/her.

Proformas to be filled by the Presiding Officer during the polling at polling booth:

1. Annexure 14 Mock poll certificate
2. Annexure 5(Part-I) Declaration by the presiding officer before commencement of the poll
3. Annexure 5(Part-II) Declaration by the presiding officer at the time of use fo subsequent voting mathcine, if any
4. Annexure 5(Part-III) Declaration by the presiding officer at the End of Poll
5. Annexure 8 Receipt Book
6. Annexure 10 Declaration by the Companaion of Blind or Infirm Elector


7. Annexure 11 (Part-I) Account of votes recorded
8. Annexure 12 Presiding Officers Dairy
9. Annexure 13 Format for presiding officer's additional report to be submitted to constituency observer/RO
10. Annexure 16 Form of Delcaration by elector whose name is in absentee/shifted/dead list
11. Form 17 A Register fo voters
12. Form 17 B List of Tender Votes
13. Annexure 20 check Memo for Presiding Officer
